Every week at Avanta Clinic in Gota, I see many patients who walk in with blood sugar levels above 250 mg/dL or HbA1c above 9%, but still, they look at me in disbelief.
Their immediate reaction is almost always the same for all:
‘Doctor, I don’t feel sick now at all. I have no pain, no extreme tiredness, and no problems working…then why do I need to take daily medication for something I can’t even feel???’
This mindset is single-handedly one of biggest reasons diabetes causes severe long-term damage before patients take diabetes seriously.
In its initial years, often for 3 to 7 years, type 2 diabetes operates silently. It does not cause obvious pain or force you into bed. But “lack of symptoms does not mean lack of organ damage. By the time, symptoms force you to visit doctor, the time for simple effective early prevention has already closed & management becomes far more complex.
Note: Patient details altered for privacy.
A 42-year-old corporate professional who was residing near vaishnodevi circle came to our clinic for routine executive health checkup which was mandatory for his job. He had no complaints, ate well, and had a active lifestyle…
However, his routine lab results showed:
Fasting Blood Glucose: 186 mg/dL
Post-Prandial Blood Glucose: 278 mg/dL
HbA1c 9.4%
When I personally reviewed his detailed history, he admitted that two years prior, a local health camp reading had shown borderline high sugar level. But Because he felt completely healthy, he ignored it and stopped testing also avoided medical consultation.
Even though he felt fine, those two years of uncontrolled blood sugar had already begun affecting his body:
Early Microvascular Changes: I ordered his eye evaluation, which revealed early stage diabetic retinopathy (silent damage to small retinal blood vessels).
Kidney Strain: Urinalysis showed microalbuminuria (early leakage of protein in the urine), signaling early stress on his kidneys.
Pancreatic Fatigue: His beta cells (the insulin-producing cells in the pancreas) were working overtime against severe insulin resistance, which we could detect by testing HOMA-IR test, rapidly losing their capacity.
If he started structured lifestyle changes and medical management two years earlier, we could have protected his pancreatic function and prevented microvascular strain altogether. But Instead, he now required immediate OHA combination therapy and strict 3 monthly monitoring.
To understand why diabetes is so deceptive, you need to understand how the body copes with excess blood glucose:
Insulin Compensation: In the early stages of Type 2 Diabetes, our body compensates for high blood sugar by producing excess insulin. Our body adjusts to elevated glucose levels & hides symptoms like extreme thirst or frequent urination.
High Renal Threshold: Your kidneys only spill glucose into your urine (causing frequent urination and extreme thirst) when blood sugar exceeds roughly 180 mg/dL. If your sugar sits between 140-175 mg/dL, you will feel zero symptoms, even though blood vessels are suffering continuous micro-damage.
Vascular Micro-Damage: High sugar damages small blood vessels (capillaries) gradually. You do not feel your nerve endings dulling or your kidney filters hardening until significant capacity is lost.
When patients finally come to the clinic because of symptoms, they usually present with:
Constant fatigue and unexplainable weight loss.
Numbness or tingling sensations in the feet (Diabetic Neuropathy)
Blurry vision
Slow-healing cuts or skin infections.
When you wait for these red flags, you are no longer treating early-stage diabetes; you are managing complications.
THE ASYMPTOMATIC DIABETES TIMELINE
YEARS 0–5: High Sugar | Zero Symptoms | Silent Damage
YEARS 5+ : Symptoms Appear | Organ Strain | Complications
Living in urban Ahmedabad, with sedentary desk jobs, high-carb dietary habits & elevated stress levels, substantially increases metabolic risk for adults over 30 age.
Here is a straightforward protocol to stay ahead of silent diabetes:
Get Screened Annually (Even Without Symptoms): If you are over 30, have a family history of diabetes or having sedentary lifestyle, get a baseline fasting Blood Sugar, post-Prandial Sugar, and HbA1cte st once a year.
Do Not Ignore “Borderline” Numbers: A Fasting Sugar between 100-125 mg/dL or HbA1c between 5.7-6.4 is pre-diabetes. This is your prime window to reverse metabolic impairment without lifelong medication.
Take Prescribed Medication Consistently: If your internal medicine physician prescribes medicines, take it to protect your organs from sub-clinical damage, not just to lower a number on a paper report.
No, never stop medication abruptly without consulting your physician. Normal sugar levels often mean that treatment is working effectively to protect your organs. Any modification or tapering must be supervised by your doctor based on repeated FBS, PP2BS, HbA1c trends.
For most non-pregnant adults, a normal HbA1c is below 5.7%. A reading between 5.7% and 6.4% indicates pre-diabetes, and 6.5% or higher on two separate tests indicates diabetes. Personal targets may vary depending on age and co-existing medical conditions.
You can undergo comprehensive blood sugar testing, HbA1c screening, and metabolic evaluation at Avanta Clinic, located conveniently in Gota, serving residents across Gota, Sola, Vandemataram, and the surrounding North Ahmedabad area.
Do not wait for symptoms to tell you your blood sugar is high. Early detection with structured medical evaluation are your BEST defense against long-term complications.
If you have not checked your blood sugar in the past 12 months, or if you were previously told your sugar was “borderline high,” schedule a comprehensive metabolic evaluation with Dr. Vivek Gurjar (MBBS, MD – Internal Medicine & Diabetologist) at Avanta Clinic.
